To ensure compliance and to set you up for prompt receipt of payment, all team members must complete the appropriate documents based on their classification. Please review the list below carefully and complete the forms that apply to your role. Links to each document will be available under your specific employment type.

Onboarding Document Submission

All required forms must be completed and submitted to [email protected] as quickly as possible to ensure compliance and avoid any delays in processing. If you have questions or need assistance while completing your paperwork, please don’t hesitate to reach out — you can email our HR team at the same address or contact Nikki directly at 407.800.1797. W-2 Employees

Form W-4 (Employee’s Withholding Certificate)

Form I-9 (Employment Eligibility Verification with required ID documents)

W-2 Employee Agreement (Must be customized to each Employee individually and will be sent directly to your email for e-signature)

Direct Deposit Authorization Form (Provided by your banking institution)

Emergency Contact Form

Licensing and Certifications Upload (if applicable, e.g., insurance license, notary commission)

Acknowledgment of Nicolette Management Handbook & Company Policies

Benefits Enrollment Forms (if applicable, sent from carriers)

1099 Contractors

Form W-9 (Request for Taxpayer Identification Number and Certification)

Independent Contractor Agreement (Must be customized to each Contractor individually and will be sent directly to your email for e-signature)

Direct Deposit / ACH Authorization Form (Provided by your banking institution)
